Parent/Guardian Declaration
I, the parent/guardian of the player whose information is provided in this registration form, hereby declare the following:
I authorize Fujairah Chess & Culture Club to register my child in the UAE Chess Federation as part of the club’s official player roster. I also authorize the club to renew this registration annually without requiring additional consent from me, unless I submit a written request to cancel or modify this authorization.
I confirm that all information provided in this form is accurate and truthful, and I assume full legal responsibility for its correctness.
I agree that the player shall comply with all internal regulations and policies of the club, including codes of conduct, professionalism, and discipline during training sessions, tournaments, and events.
I acknowledge that the club reserves the right to take appropriate measures in case the player violates any applicable rules, regulations, or policies.
